Caring for Homebound Elderly Patients

This workshop evolves from the case of a frail, homebound, geriatric patient with complex comorbidities, unable to access traditional community-based services, and impacted by the Covid 19 pandemic restrictions.  The interprofessional faculty facilitates exploration of the psychosocial, nursing, oral health, medical, rehabilitation, nutritional, spiritual, neuropsychiatric, and public health issues raised in this case. Students participate in shared learning and problem-solving small group breakout sessions. The emphasis centers on the role of teamwork, understanding and use of community and clinical resources, treatment planning, improvement in quality of life, and the challenges in the delivery of care in the home.

2:00 – 2:15 pm (15min) | Welcome (Mark Nathanson)

2:15 – 2:30 pm (15min) | Breakout Rooms Part 1 (small groups)
• students will break into 20 teams of 10 to read part 1 of the case and answer part 1 discussion questions and respond to Polling question 1
2:30 – 2:45 pm (15min) | Case Discussion Part 1 (large group)
• Mark will facilitate panel discussion as well as small group report out and student questions & comments
2:45 pm – 3:00 pm (15min) | Breakout Rooms Part 2 (small groups)
• students will re-enter same breakout rooms to read part 2 of the case and answer part 2 discussion questions
3:00 – 3:3:15 pm (15min) | Case Discussion Part 2 (large group)
• Mark will facilitate panel discussion as well as small group report out and student questions & comments
3:15 – 3:30 pm (15min) | Large Group Wrap-Up, Future Considerations & Final Thoughts (large group)
